Integrated Care Resources now available via online portal

As part of the West Midlands Integrated Care Transformation Theme we are proud to announce
the launch of a range of resources to support Integrated Care via the HEE West Midlands
Learning Portal.
Health Education England has identified integrated care as one of our key priorities in the West
Midlands. We are committed to supporting our workforce to deliver health and care services that
are both person centred and well-coordinated.
We have recently identified a set of valuable tools and resources which can be used by care
workers and service commissioners and many of these will also support unpaid carers and
citizens.
Two aspects that support Integrated Care are; self-care and resources for carers (paid and
unpaid), the portal brings together a wide range of resources via one easy to access page.
